Target Population Demographics
We know that health disparities can be a major barrier to achieving optimal health outcomes, which is why we have made it our priority to address these issues head-on. By leveraging the electronic health record and the annual UDS report, we are able to track health disparities in the Pasadena community and make data-driven decisions to improve health outcomes.
We are committed to providing compassionate care to everyone who walks through our doors, and we will continue to work tirelessly to ensure that our community receives the best possible care.
